You are a nurse with an affinity for patients facing addiction and psychiatry issues. Together with your colleagues, you want to offer perspective to these patients. Would you like to apply and share your knowledge and skills within the Medical Service outpatient addiction care location(s) Venlo/Venray? We are looking for an enthusiastic, strong personality to strengthen our team.

Your tasks are diverse. You get to know a patient during an intake interview, which you conduct independently. Together with the addiction physician, you look for a suitable trajectory for the patient, after which you make an active contribution to the process of indication, MDTs, up to starting and executing a treatment. You have your own caseload and coordinate care in the role of trajectory supervisor. You are the point of contact for the patient and support them in their treatment. We often deal with complex issues, where multiple disciplines are called in if necessary, such as psychologists, nurse practitioners, or the psychiatrist. There is close cooperation with your team and the involved addiction physician. The population we see consists mainly of patients who are dependent on opiates. Daily, we see patients at our location for methadone dispensing.

In addition, you will regularly have contact with our chain partners, such as the Salvation Army, the municipality, police, and other healthcare providers involved with the patient. These coordinating tasks involve a degree of case management.

We provide daily medication dispensing, both at our location and at Domus Venlo, part of the Salvation Army. We often work from a 'Harm Reduction' perspective. You work regular shifts, organize your own workday, and work both independently and in a team.

You will work in a multidisciplinary team with nurses, an addiction physician, and a nurse practitioner. Additionally, there is an overarching team with SPVs, basic psychologists, GZ-psychologists, and a psychiatrist.
