About VIGO
In their own words
About the company & team
Our tasks are diverse. Following an initial registration, an introduction with the patient takes place during the intake. You will conduct this conversation independently, after which you will work together with the performing treating practitioner to find a suitable trajectory for the patient. Following this, as the patient's trajectory coordinator, you will make an active contribution to the process of indication, MDTs, through to starting/executing the treatment.
As a trajectory coordinator, you will maintain your own caseload where you are the point of contact throughout the treatment trajectory and ensure coordination for the necessary care.
This coordinating role requires you to be able to maintain/monitor the overview, and to be connecting and collaborative with both the patient and the involved healthcare professionals. Where necessary, multiple disciplines are brought in, such as: addiction physicians, nurse practitioners, psychiatrists, or psychologists. You will also regularly have contact with our chain partners such as: Salvation Army, Moveoo, municipality, police, and other involved care providers.
Daily, we see a portion of our patient population at our dispensing locations in Venlo/Venray. The largest part of the target group is dependent on methadone and comes to our clinic for opioid replacement therapy.
In this, we often work from a harm reduction method for both the patient and society called 'Harm Reduction'. With this method, we work from a holistic approach.
In our work, we deal with complex issues because our target group often does not have multiple areas of life in order. Despite the fact that you, as a healthcare provider, map out a suitable trajectory together with the patient and create possibilities for recovery, the patient may make choices that do not align with that. This sometimes requires adjusting expectations and goals, without losing sight of hope or perspective. You are that nurse who can slow down and endure, see opportunities, and can leave the control with the patient without losing the connection.
You will divide your own workday between dispensing moments and work both independently and in a team.
