The role

For our Mental Health Care in the Neighborhood team (Lelystad), we are looking for a Personal Outreach Support Worker for at least 24 hours per week (hours in consultation).

You guide them toward an independent and safe existence, working from a foundation of trust and with an eye for the person behind the behavior. You help clients find and maintain a stable living and residential environment and, together with them, set goals aimed at recovery and self-reliance. In doing so, you offer practical support, for example with debts, administration, or arranging housing. You also guide clients to appointments at agencies such as the court, Probation Service, or practitioners. You are the connecting link between the client, colleagues, and external organizations such as municipalities, police, and healthcare institutions. You actively involve the client's social network in the recovery process. You adapt when necessary: sometimes you are a coach, sometimes a facilitator, sometimes a listening ear. You work with people and with their stories, where cultural sensitivity is essential. No two days are the same. You work independently, but you are never alone. You are given space to be creative in your approach and to grow alongside your clients. The impact of your work is directly felt. For the client, their environment, and for you.

Your workplace

Our GGZ in the Neighborhood team in Lelystad consists of dedicated colleagues and offers customized support to young adults, adults, and families with a psychiatric or psychological vulnerability, possibly in combination with a mild intellectual disability and/or substance use. The team works with support programs such as Kamers met Kansen, Housing First, Bemoeizorg, and Gewoon Thuis, among others.

We offer both planned and unplanned support within a structure of 24-hour availability and accessibility. The 24-hour accessibility means that employees fulfill a telephone on-call service at least three times per month, running from 08:00 to 19:00. We work closely with our own team, the network, the neighborhood, and various chain partners. We maintain an open and approachable attitude, see obstacles as challenges, and make optimal use of everyone's unique expertise.
