You have a completed HBO degree in social work (or comparable);
Experience or affinity with the target group is a must;
You are available for 24 hours per week;
You possess a B driving license;
You possess good coordinating skills, are proactive, flexible, and possess a high degree of independence;
Your observational skills and good communication skills ensure that you can master unforeseen situations;
To be eligible for this position, a positive Certificate of Good Conduct (VOG) is required;

What you'll get

We offer you a market-conform salary CAO GGZ FWG 55. The salary amounts to a minimum of € 4,000 and a maximum of € 5,294 for a full-time employment, depending on your knowledge and experience;
A permanent contract for 24 hours per week. We believe in sustainable cooperation, therefore, upon mutual satisfaction after a 2-month probationary period, we offer you an employment contract for an indefinite period.

About the company & team

The social work team is looking for a new colleague for the Continued Treatment & Residence cluster (long-term care) starting March 1, 2027. The team consists of five colleagues who each have their own caseload but work closely together. You can always rely on each other for advice, consultation, or sharing information.

No two days are the same in your work. You coordinate the integral process flow of psychosocial treatment and guidance around assigned clients, whereby you stimulate the clients' own autonomy. Where necessary, you provide support or guidance, or you take over tasks. In this, you continue to adopt a coaching role to strengthen self-management and increase problem-solving abilities. You coordinate the support with practitioners and the nurse responsible for care coordination. Where necessary, you liaise with (external) disciplines. You also organize things such as family evenings and serve as the first point of contact for psycho-education.

As a social worker, you participate in the MDO (Multidisciplinary Consultation). You contribute to policy and social levels from the client's perspective. It is important that you know psychiatry and target groups and can assess where the underlying request for help originates.
