Social Worker FACT Elderly Amstelveen

At GGZ inGeest, you will work as a social worker in the FACT team Elderly Amstelveen. Together with a multidisciplinary team, you provide recovery-oriented care to clients with severe psychiatric disorders. • The FACT elderly teams focus on the treatment and recovery of people with severe psychiatric disorders (EPA) over the age of 65 with serious problems and dysfunction in multiple life domains. Elderly people with cognitive disorders, where psychiatric comorbidity plays a role, are also treated. If necessary, the family is involved in the treatment.

• FACT elderly offers long-term care-dependent psychiatric clients treatment and support in all kinds of life areas in their own environment outside of the hospital. As a result, severely dysregulated chronic psychiatric clients can also live independently in the neighborhood (or in some cases in a RIBW) with the support of multidisciplinary FACT teams.

• Additionally, we support (re)participation in society. In this, we work together with other organizations to support the client as well as possible. The client receives extra help if necessary, and less help when possible.

• Work is carried out according to the F-ACT method: an intensive form of outpatient community-based treatment from a recovery-oriented perspective by various people working together in one team. Our starting point remains to find connection with the goals and wishes of the client.

• Within FACT Elderly, work is performed in multidisciplinary teams consisting of psychiatrists, specialists in geriatric medicine, nurse practitioners, mental health psychologists, general practitioners, outpatient nurses/coaches, social workers, and trajectory coordinators.

• In close collaboration with fellow practitioners, you take charge of the clients in your caseload. This includes, among other things, home visits and appointments with the client on location, where the focus lies on improving their social participation.

• You guide and support clients with practical and social requests for help, such as financial matters, housing, and daily activities. Additionally, you identify changes in the client's situation and respond adequately to these by deploying appropriate interventions together with the team.

• You stimulate the self-reliance and autonomy of clients through motivational interviewing and solution-oriented guidance, and you involve family and loved ones in the treatment process to ensure a supportive network.

• You are empathetic and willing to listen, maintaining an eye for the client's full story. Together with them and their loved ones, you strive for recovery, no matter how small the steps may sometimes seem. 

• Actively collaborating with chain partners such as general practitioners, district nursing, housing associations, and municipal authorities to realize the best possible care is an important part of your duties.

• You contribute to the drafting and evaluation of treatment plans in collaboration with the FACT team.
