What you'll get

As a Dementia Support case manager, you will be scaled according to the VVT collective labor agreement, FWG 50 or FWG 55, with a maximum salary of €4,822.50 or €5,413.38 gross per month based on a 36-hour work week. The position is for a minimum of 24 hours per week.

The role

Caring for people with dementia? We do that together. Will you make the difference for your clients through attention and coordination as a case manager?

What you'll do

As a case manager, you are the central face within the network surrounding the client. From an independent role, you represent the interests of both the person with dementia and their loved ones. You follow the entire trajectory from A to Z: you advise, organize, and coordinate all the care and support that is needed. In doing so, you work closely with professionals within the network and know how to connect them to their responsibilities in a natural way. With your professional eye, warm personality, and methodical way of working, you bring clarity to complex situations and offer exactly that support that makes the difference.

You stand beside the client and their network. You guide, support, and actively think along. Also when informal caregivers or healthcare providers need extra advice or direction. You know how to connect parties with each other and address them when necessary. One network, one goal: working together to provide the best possible support for your client. That is what you strive for every day.

What will you do?

You will be working within Dementia Support in the Central Limburg region.

At www.hulpbijdementie.nl you can see which organizations participate in Dementia Support Central Limburg.

The role

Initially, you will receive a contract for one year, with a view to extension upon mutual satisfaction. Are you employed by one of the network partners? Then you will retain your permanent contract with your own organization, including a guarantee of return (not necessarily in the same position).

You are a creative and innovative case manager with a high degree of independence. You already have experience working in/with a network organization and possess excellent communication skills. Your field is dementia; you already have experience and affinity with this, you have knowledge of the social map, and you can effectively bridge conflicting interests. Dementia case management? You are already familiar with this and act from this understanding methodically, systematically, and evidence-based, following a cyclic process in a multidisciplinary setting.

Requirements

  • Supplemented with a post-HBO dementia case management training or you are willing to follow one

  • Due to the ambulatory nature of your position, you possess a driver's license and a car
