What we're looking for

Additionally, you have completed the post-HBO Dementia Casemanagement training or are willing to follow it. The aforementioned bachelor's degrees are a requirement for this position. Therefore, we unfortunately cannot include candidates with a different educational background in the application procedure.

Due to the ambulatory nature of the position, you possess a valid driver's license and have access to a car.

Education & certifications

To be able to work as a dementia casemanager, you possess one of the following completed bachelor's degrees:

  • HBO-Nursing;

  • HBO-Social Work (including MWD and SPH);

  • HBO-Applied Psychology;

  • HBO-Applied Gerontology.

The role

Working together to provide appropriate support, close to the person with dementia and their loved ones

In the North Limburg region, various organizations participate within the Dementia Support chain. The goal is to offer a coherent and coordinated whole of care and services to people with (a suspicion of) dementia and their loved ones, allowing the person with dementia, if desired, to continue living at home for as long as possible with the highest possible quality of life. Support with activities and social contacts is also included in this. The personal wishes and needs of the person with dementia and their loved ones form the starting point. They have the most important voice in this process. To ensure quality, unambiguous agreements have been made regarding standards, working methods, tasks, and responsibilities. All participants work according to these agreements.

On www.hulpbijdementie.nl you can see which organizations participate in Dementia Support in the North Limburg region.

The role

As a dementia casemanager, you are the central face within the chain surrounding the client. From an independent role, you represent the interests of both the person with (a suspicion of) dementia and their loved ones. You follow the entire trajectory from A to Z: you advise, organize, and coordinate all necessary care and support. In doing so, you work closely with professionals within the chain and connect them to their responsibilities in a natural way.

With your professional insight, warm personality, and methodical way of working, you bring clarity to complex situations and offer the support that makes the difference. You stand alongside the client and their network. You guide, support, and actively participate in thinking along, even when informal caregivers or healthcare providers need extra advice or direction. You connect parties and hold them accountable when necessary. One network, one goal: working together to provide the best possible support for your client. That is what you strive for every day.

Requirements

You are an independent and committed professional who enjoys collaborating and is simultaneously well able to organize your own work. You think creatively and seek appropriate solutions, even when situations are complex. You have an affinity with and preferably experience in the field of dementia. You are familiar with the social map or know how to master it quickly. You can also handle different, sometimes conflicting, interests of clients, loved ones, and involved professionals effectively.

  • You are good at planning and organizing.

  • You possess strong communication and digital skills.

  • You work methodically, systematically, and evidence-based.

  • You can make decisions independently and bear responsibility.

  • You work easily with different disciplines and organizations.

  • You know how to create connections between the client, loved ones, and professionals.

Our offer

You will be rewarded according to the VVT collective labor agreement (cao). As long as you have not yet completed the post-HBO Dementia Casemanagement training, you will be scaled in FWG 50, with a salary up to a maximum of € 4,822.50 gross per month. After obtaining the required diploma, you will be scaled in FWG 55, with a salary up to a maximum of € 5,413.38 gross per month. The mentioned amounts apply to a full-time employment of an average of 36 hours per week. You will work a minimum of 24 hours per week. We will start with an employment contract for one year, which will be extended upon mutual satisfaction.
