About VIGO
In their own words
About the company & team
The Korsakov center for alcohol-related disorders, part of the Top-Referral departments within Vincent van Gogh for mental health, is looking for a driven care coordinator like you!
Are you ready to take on a new challenge with us? Do you want to be part of an enthusiastic and committed team? Then read on quickly, or even better, send us a response immediately?
Our center focuses on the diagnosis and treatment of people with (the suspicion of) alcohol-related cognitive disorders. Complicating factors include serious socio-economic problems, (self)neglect, apathy, and a lack of insight into the illness. Our diagnostic and treatment trajectories can take place both clinically and outpatient.
The center consists of several departments including:
Korsakov treatment
Korsakov admission and diagnostics
Cognitive treatment (resocialization)
Korsakov living
Outpatient at home
Each department has its own expertise and area of focus. All departments are closely connected and work extensively together with each other and with the treatment team. This treatment team consists of, among others, clinical neuropsychologists, psychiatrist, social work, nurse practitioners, and MBO and HBO nurses and caregivers. The goal of Korsakov Living is to enable clients to function as well as possible in daily life again. The client population of Korsakov living consists of 13 clients with severe cognitive damage or with Korsakov syndrome. From a positive perspective, looking at possibilities, you work together with the client towards the most pleasant existence possible. You conduct conversations about physical and mental functioning and well-being, support clients in ADL, and stimulate their participation in daily activities.
As a care coordinator (ZOCO), you take charge of the care process within the team and support healthcare providers (EV'ers) where necessary. You organize and coordinate the daily care and division of work of employees within your own team, monitor the quality and progress of care, and resolve bottlenecks. You are the point of contact for the department, make decisions when necessary, and monitor agreements made. Additionally, you give functional instructions and/or guidelines to team members and manage them operationally regarding care content.
You work closely with the lead practitioner (from whom you receive functional instructions), the expertise leader, and the team leader (to whom you receive hierarchical leadership). Together with them, you draw up an annual plan in which goals, quality control, and quality assurance are established. You evaluate work agreements, coordinate activities during handovers, and ensure clear communication within the team.
Your areas of responsibility and core tasks include, among others:
Monitoring the completeness of files and signaling irregularities
Promoting quality and expertise within the team
Coordinating intervision, case discussions, and clinical lessons
Organizing appropriate care around the client at the case level with other disciplines
Drafting and updating work processes, checklists, and treatment components
Supervising the content and proportionate distribution of healthcare provider shifts
Representing the team in consultation structures at clinic, department, and client levels.
