Care Coordinator The Hague - 16 to 32 hours per week

We are immediately looking for an enthusiastic Care Coordinator for 16 to 32 hours per week. The location for this position is The Hague, but you are willing to work at our Gouda and Leiden locations when necessary.

Job Description
As a care coordinator, you are the first point of contact for everyone undergoing treatment with us and you are responsible for the entire logistical and administrative process of the treatment trajectories, from registration through to completion. An affinity with the telephone, planning, and administration must be part of your DNA, as this constitutes a large part of your responsibilities. Additionally, you inform patients and clients who are exploring one of our treatments in a pleasant and professional manner. You receive patients and clients during screenings or intakes and handle the correspondence regarding the entire trajectory. In short, you are our social multi-tasker.

You
You possess MBO/HBO working and thinking levels and have several years of relevant work experience in an administrative or commercial role. You are also organizationally and administratively strong. Additionally, you have experience in providing information in various ways to patients, GPs, and other target groups, for example through presentations. Ideally, you have basic knowledge of morbid obesity and motivational interviewing. You perform your work proactively, enthusiastically, and you maintain an overview easily. You are digitally skilled and have extensive telephone experience. Integrity, discretion, and tact are second nature to you. Work experience in healthcare is a plus. Finally, you enjoy working at different locations.
