The role

description

Do you want your own caseload and control over the care you coordinate?

This assignment lasts six months with the possibility of extension, 24 to 36 hours per week, from one team location in Gorinchem. You will become part of the team. No changing faces, no constant re-onboarding.

You will hear from us within one working day. Always, even if it is a no.

It saves us both time: without a completed diploma in healthcare or welfare, we cannot do anything for you. Our clients simply will not place you in a group then. Which diploma you need is listed under Is this for you. A foreign diploma counts as soon as Nuffic or IDW has evaluated it.

What you will do

The activities take place within a FACT team, Flexible Assertive Community Treatment, where outpatient treatment, rehabilitation, and recovery are central for clients with severe psychiatric disorders. This target group is characterized by a long-term and fluctuating course of symptoms, often in combination with limitations in social and societal functioning. Within the team, intensive collaboration takes place across different disciplines, with the aim of supporting clients in recovery and maintaining as much self-management as possible.

As a casemanager, you are responsible for the coordination of care and guidance of your own caseload. You work both outpatients and from a team location.

  • Conducting home visits and contact moments with clients.
  • Coordinating care and acting as a point of contact for clients.
  • Identifying changes in the clinical picture and anticipating them.
  • Guiding and treating clients within a recovery-oriented approach.
  • Participating in daily meetings and multidisciplinary coordination.
  • Maintaining contact with practitioners, networks, and external organizations.
  • Ensuring reporting and file management.
  • Utilizing your own expertise and continuing to develop through professional development.

You build a bond with the same clients and you see the results of your guidance. You work according to a schedule that you know in advance.

You will be employed on a secondment basis by GOED. and work for the client. Your salary, your contract, and your guidance are managed through us.

What you earn

  • Salary in accordance with the CAO Mental Health Care, job group 50: €3,560 to €4,716 gross per month for 36 hours. Amounts as of July 1, 2026. Scaling depends on your diploma and your experience.
  • 8.33% holiday allowance, 8.33% year-end bonus, and irregularity allowance: 22% in the evening, 44% at night, 38% to 49% on Saturday, and 60% on Sunday and public holidays. A Sunday shift therefore yields up to €44.43 per hour.

How it works when you apply

  1. You submit your application. Short and powerful is fine.
  2. You will hear from us within one working day.
  3. You schedule your own online introduction, at a time that suits you.
  4. In a follow-up interview, we will discuss your wishes and your experience.
  5. You will get access to your own GOED. environment.
  6. You receive your agreement, in black and white.
  7. You choose your first shift.

What else you get

  • Your BHV (Emergency Response), administering medication, and aggression regulation are free of charge, provided by GOED.academy.
  • A contract with fixed hours and a schedule that you know in advance.
  • One permanent position, so that you truly get to know your clients.
  • Holiday pay, vacation days, travel expense reimbursement, and pension accrual.
  • If this assignment ends, we will look for the next one. You do not need to apply again.

Does this suit you?

  • A completed HBO (higher professional education) degree in healthcare or welfare, preferably HBO nursing or a completed HBO degree in SPH, MWD, or Social Work with an outflow profile in Care or Long-term complex care.
  • Extensive experience with FACT working and affinity with clients with severe psychiatric disorders.
  • You can work independently, are strong in coordination and communication, and switch well in a dynamic environment where crisis situations may occur.
  • A recovery-oriented vision, creativity, and the ability to leave control to the client.
  • A calm and professional attitude, even under pressure, with which you offer clients the necessary peace and security.
  • Experience in FACT is a plus.
