Together with the client, you draw up the support plan and keep a close eye on progress and goals. You coordinate the support, coordinate with colleagues, and maintain contact with the network and other involved professionals. In doing so, you constantly look at what the client needs to live and reside as independently as possible. You are also involved in intakes with new clients, you think along about whether it is appropriate within the team, and ensure that the support is started. In addition to your coordinating role, you naturally also provide support yourself. You are involved in the daily support and know how to act calmly and make the right choices in unexpected situations. You will join a committed team that is fully in development. We try to create a regular schedule for the coordinating support workers. Because the team is not yet completely full, this may not always be possible in the beginning. At this moment, you will also work on-call shifts in the evening and sometimes work on weekends. Together, we continue to build a stable team and a beautiful form of home-based support.
