What you'll get

We have the basics in order, and more than that. Simply because we believe you should be well taken care of.

  • A one-year contract for 24–32 hours per week, with the intention of converting this to a permanent contract.

  • A salary in scale 55/60 (FWG 3.0) of at least € 3,903 and at most € 6,098 per month based on 36 hours per week, depending on education and experience.

  • 8.33% holiday allowance and an end-of-year bonus of 8.33%: a full thirteenth month.

  • Pension accrual at Pensioenfonds Zorg & Welzijn, travel expense reimbursement, and a pleasant staff association.

  • The Flexible Benefits System (MKA), including a bicycle plan, company fitness, the option to purchase extra vacation days, and a discount on your health insurance premium.

  • Plenty of room to grow via the St Jansdal Academy, (online) and at your own pace. Plus regular workshops and initiatives to keep you healthy and vital.

The role

You get energy from connecting people, organizations, and care processes. Today you are sitting at a table with GPs discussing a bottleneck in the chain; tomorrow you are working with medical specialists and management to establish new working agreements. No two days are the same, and that is exactly what makes the work so enjoyable for you. Because ultimately, it all comes down to one thing: the best care for the patient, in the right place. Simply because care is in our blood here.

Why this position suits you

In this role, you combine relationship management, advisory, process improvement, and project-based working, with plenty of room to take the lead yourself. You oversee the bigger picture, bring different interests together, and translate complex issues into concrete solutions. And you ensure that agreements do not just stay on paper, but truly work in practice.

With your listening skills, analytical ability, and decisiveness, you get people on board and set improvements in motion. In this way, you make a visible impact on the collaboration between healthcare organizations in the region.

The Transmural Coordinating Center (TCC)

St Jansdal is the beating heart of healthcare in the region, and that role does not stop at the hospital's front door. The TCC is the connecting link between GPs, medical specialists, and other healthcare partners in the region. We stimulate uniform agreements, better coordination, and high-quality care, always from the patient's perspective. Thanks to the TCC, healthcare providers can count on smooth referrals, clear collaboration agreements, and effective regional coordination.

The four of you keep the TCC running: You know each other's work, step in where necessary, and work together to ensure everything runs smoothly. The TCC is undergoing significant innovation. This calls for someone who wants to help build, and it gives you the chance to leave your mark on how we further shape collaboration in the region.

What you'll do

What you will do

  • You move daily between GPs, medical specialists, and the hospital. You keep an eye on multiple files at once and switch gears quickly when a situation demands it. Your responsibilities include:

  • Being the fixed point of contact for GPs and healthcare institutions in the region. You actively build these relationships, go on visits, know what is happening, and ensure that questions and signals reach the right place within the hospital.

  • Managing your own portfolio of healthcare partners. You invest in those relationships and take the initiative yourself to improve collaboration.

  • Initiating, developing, implementing, and keeping regional transmural working agreements (RTA's) up to date.

  • Identifying bottlenecks in collaboration, analyzing what is happening, and advising on solutions and improvements.

  • Starting up and guiding projects, network initiatives, and joint improvement trajectories, providing process support and substantive advice.

  • Contributing to regional transformation plans, aimed at providing the right care in the right place.

  • Organizing and coordinating meetings, such as GP meetings and regional gatherings.

  • Representing St Jansdal in regional and national networks, including the NTSN. You feel at home at every consultation table: from operational to administrative levels.

  • Translating complex agreements, developments, and diverse interests into clear communication and workable solutions.

What you bring

You enjoy building bridges between people, organizations, and processes. You think in solutions, take responsibility for advancing initiatives, and reduce complex information to clear advice and concrete actions. You work independently, but actively seek collaboration when necessary. And you never lose sight of the collective goal. You also bring:

  • A completed HBO (higher professional education) or WO (university) degree, supplemented by relevant training or courses.

  • A desire to take the lead. You seek people out yourself, dare to name bottlenecks, and maintain control until agreements truly work.

  • Experience in a coordinating, advisory, or policy-making role, preferably in healthcare, and a genuine interest in the collaboration between primary and secondary care.

  • Experience with stakeholder management and a sense for administrative relationships between GPs, medical specialists, and the hospital organization.

  • Strong communicative, analytical, and organizational skills. You switch easily between different levels and perspectives.
