Healthcare Mediator

• a higher professional education (hbo) degree in Healthcare and Welfare, Nursing, or Social Work;

• knowledge of the Dutch healthcare system and of indication setting, Wlz, Zvw, and Wmo;

• experience with network- or area-oriented working;

• knowledge of medical conditions, medication effects, and the degree of self-reliance;

• good digital skills and experience with registration systems;

• preferably experience with healthcare mediation, care coordination, or a similar position within healthcare or welfare;

• preferably knowledge of Rotterdam and the healthcare and welfare network in the city;

• preferably a relevant quality registration and/or BIG registration.

Additionally, you are willing to work from home periodically outside of office hours.

• an employment contract for 32 hours per week, initially for 12 months with the intention of a permanent employment contract;

• a salary according to FWG 50 of the VVT collective labor agreement: € 3,592.96 to € 4,822.50 gross per month based on 36 hours;

• an end-of-year bonus of 8.33%;

• good pension accrual via Pensioenfonds Zorg en Welzijn (PFZW);

As a Healthcare and Welfare Mediator, you are the first point of contact for clients, informal caregivers, referrers, healthcare organizations, and colleagues with a healthcare or support request. You map out the situation, perform telephone triage, and assess which care or support is the best fit.

You are familiar with the possibilities within the Wlz, Zvw, and Wmo and investigate which funding and which care provision are appropriate. In doing so, you look beyond only the offering of Humanitas: appropriate care for the client is the priority. You monitor the progress of applications, register your actions carefully, and ensure a good handover to the organization that will provide the care.

Additionally, you are active within the Rotterdam healthcare and welfare network. You maintain contacts with, among others, general practitioners, hospitals, CIZ, welfare organizations, and other healthcare providers. You are regularly present at various locations and in the neighborhoods to bring attention to Humanitas and to further strengthen the cooperation with partners.

• mediate in urgent and non-urgent care requests;

• advise clients, informal caregivers, and referrers about appropriate care and support;

• liaise with healthcare providers, referrers, and internal teams;

• maintain an overview of multiple applications and monitor progress;

• signal bottlenecks in the care provision and discuss these with the parties involved;

• actively build relationships within the Rotterdam healthcare and welfare network.
