The role
Are you an experienced outpatient nurse/supervisor with a passion for mental healthcare and providing high-quality care? Do you want to work flexibly and apply your expertise within different teams at GGZinGeest? Do you enjoy being a central figure and are you good at planning and organizing? Then we are looking for you!
We ask flexibility from you and offer a suitable contract in return. With a min-max* contract, you have the freedom to work more hours during busy periods and reduce your working hours during quieter times, which ensures a good work-life balance. You appreciate that not everything is fixed and that you can flexibly schedule your working days in consultation. Moreover, you get the chance to work in different FACT teams, which means you will deal with diverse care issues and your work remains varied and challenging every day! Interested? Apply directly!
*We work with a min-max contract between 24 to 36 hours. In this case, you will be deployed for a minimum of 24 hours; the remaining hours are in consultation.
Appropriate care for your clients
The FACT teams of GGZ inGeest treat adults (up to 65 years old) or elderly people (65+) with a severe psychiatric disorder (SMI) and care needs in various areas of life, such as living, working, meaning, health, finances, and social contacts.
Work is carried out according to the F-ACT method: an intensive form of outpatient, community-based treatment from a recovery-oriented perspective by various colleagues working together in one team. Treatments take place in the client's own environment as much as possible.
Active efforts are always made to collaborate with the client, their loved ones, and chain partners. Our starting point remains to find alignment with the client's goals and wishes.
Within FACT, work is performed in multidisciplinary teams consisting of psychiatrists, nurse practitioners, mental health psychologists, general practitioners, outpatient nurses/supervisors, social workers, IPS job coaches, and case managers.
The teams have short lines of communication with our chain partners and colleagues from other departments within GGZ inGeest.
A suitable working environment for you
We offer you a flexible and varied job, where you fly in to the place where it is needed. This is within the FACT teams of GGZ inGeest.
As an outpatient nurse/supervisor, you are deployed when there is a temporary need for extra support within a team. The duration of how long you help a team may vary; this can be from 2 months to 8 months, which you will evaluate every 3 months with your manager.
In close cooperation with your colleagues, you take (shared) control over the clients in your caseload. This includes, among other things, home visits and appointments with the client on location.
You handle various aspects of the treatment process, ranging from intake interviews to discharging—and working with—the client. And everything in between, such as managing crisis situations and working on the client's treatment goals.
You support the client in their home situation.
An important aspect of your role is initiating and maintaining the network around the client. You work closely in the community with colleagues from sheltered and assisted living, welfare organizations, neighborhood teams, the police, and the housing association.
Also important
You have a BIG registration as an HBO nurse, a completed education in HBO social work & services, HBO social pedagogical assistance, or HBO social work (outflow profile Care). It is also possible to apply with a completed post-HBO education as a GGZ-agoog.
Suppose you do not have a BIG-registered profession; we would be happy to talk with you about the possibility of becoming authorized and competent in nursing technical procedures, which in a FACT team consist of injections and medication administration.
You are empathetic and can listen like no other, keeping an eye on the client's full story.
You can shape your work well independently and are able to switch easily between different situations, including crisis situations.
You have mental healthcare knowledge and expertise and are familiar with various issues occurring within the SMI target group.
You have several years of experience within a FACT team in mental healthcare.
You own a car.
The position is scaled in FWG 55 CAO GGZ.
You will immediately receive a permanent employment contract (min-max).
At GGZ inGeest, we support you in your development, whether it is about deepening expertise, learning new skills, or increasing digital knowledge. You can read how we do this in the general terms and conditions.
