Requirements
What we're looking for
You have a (nearly) completed education as a mental health nurse specialist and are BIG-registered.
The role
Discover a dynamic role as a lead practitioner at GGZ inGeest. Work multidisciplinarily and outreach-oriented with clients with severe psychiatric disorders. Support recovery and develop further in a dynamic working environment. Your heart, knowledge, and experience are urgently needed.
Appropriate care for your clients
The FACT Elderly team Amstelveen from GGZ inGeest focuses on the treatment and recovery of people aged 65 and older with severe psychiatric disorders (SMI), who struggle with complex issues and dysfunction in multiple areas of life. Elderly people with cognitive impairments in combination with psychiatric problems are also treated within FACT Elderly. Where necessary, the family is actively involved in the treatment.
FACT elderly offers treatment and support in all sorts of areas of life in their own living environment to long-term care-dependent psychiatric clients. As a result, even severely dysregulated chronic psychiatric clients can live independently in the neighborhood (or in some cases in a RIBW) with the support of multidisciplinary FACT teams.
In addition, we support clients in (re)participating in society. We work closely with other organizations in this regard, so that every client receives the right support: intensive when necessary, more at a distance when possible.
We work according to the F-ACT methodology: an intensive, outpatient, and community-based form of treatment, in which different disciplines work together in one team from a recovery-oriented perspective. Our starting point remains to find connection with the goals and wishes of the client.
Our multidisciplinary teams consist of, among others, a psychiatrist, a GZ-psychologist, doctors in training to become psychiatrists or specialists in geriatric medicine, outpatient nurses and supervisors, and an expert by experience.
A suitable working environment for you
As a nurse practitioner at FACT, you always work collaboratively. This means you work closely with the multidisciplinary team and referrers. You participate in multidisciplinary team meetings and treatment plan discussions.
Client support is an important part of your work. This includes regular home visits to support clients in their own living environment and to promote self-reliance and self-management. As a specialist, you provide acute care and interventions during crisis situations and work proactively to prevent crisis through early signaling and intervention.
You work as a lead practitioner and deal with, among other things, care coordination, diagnostics, the drafting and evaluation of treatment plans, and co-shaping treatment, including medicinal interventions.
As an expert, you provide consultation and advice to other team members, such as psychiatrists, outpatient nurses, and social workers, regarding complex cases.
You share your knowledge and offer support in the development and implementation of treatment strategies and actively contribute to developments in policy and quality of care.
Also important
You will receive a permanent employment contract immediately.
The position is scaled in FWG 65 CAO GGZ.
