Case Manager FACT Elderly Amstelveen

Become a case manager in a FACT team where your perspective truly counts. You guide clients in finding suitable day activities: a place where they can build structure, self-esteem, and perspective. You do this together with a multidisciplinary team of specialists, nurses, and caregivers. You stand close to the client, see what is happening, and signal what is needed, and it is precisely those observations that make the difference within the team.

• The FACT team treats elderly people (aged 65 and over) with a severe psychiatric disorder (SMI) and support needs in various areas of life, such as housing, day activities/social participation, meaning, health, self-reliance, finances, and social contacts.

• Work is carried out according to the F-ACT method: an intensive form of outpatient, community-based treatment from a recovery-oriented perspective by various people working together in one team. Treatments take place in the client's own environment as much as possible.

• Active efforts are always made to collaborate with the client, their loved ones, and chain partners. Our starting point remains to find alignment with the client's goals and wishes.

• Within FACT, work is performed in multidisciplinary teams consisting of psychiatrists, nurse practitioners, mental health psychologists, general practitioners, outpatient nurses/caregivers, social workers, and case managers. The team has short lines of communication with our chain partners and colleagues from other departments within GGZ inGeest.

• As a case manager, you are part of the treatment team and, in addition, have close contacts with other organizations such as welfare organizations. 

• You address questions from the team regarding clients' wishes for day activities, expanding social networks, and increasing self-reliance. Additionally, you perform occupational therapy assessments upon request.

• You visit clients at home or meet at a place in the neighborhood they find pleasant or that is helpful in building activities outside the home. Together, you investigate the support needs and draw up an action plan to achieve their goals. You provide sincere attention to every client while simultaneously maintaining a professional attitude. The client's own autonomy is central to your work, and you align with their needs.

• You are always actively looking for places where clients feel at home and can participate in activities. You build a network of community centers and welfare organizations so that you can easily provide clients with day activities.

• The position is scaled in FWG 50 cao GGZ.
