Nice to have
This job advert was originally written in Dutch.
You have a BIG registration as an MBO nurse level 4.
Experience in GGZ is a plus.
In collaboration with your colleagues in the FACT team, you offer recovery-oriented care to clients with severe psychiatric disorders. You develop yourself in a learning environment and contribute to future-oriented care. Your heart, knowledge, and fresh perspective are more than welcome.
For our FACT teams, we are starting with the (re)introduction of the community MBO nurse. In the coming period, as part of a pilot, we will work together with you to properly shape the embedding of this role within our FACT teams. And we are preparing the possibilities to grow into an HBO nurse. Are you ready for a next step and are you looking forward to contributing to the treatment of people with an EPA care indication together with the colleagues in our FACT teams? Then please contact us.
The FACT teams treat adults and the elderly with a severe psychiatric disorder (EPA) and support needs in various areas of life, such as housing, work, meaning, health, finances, and social contacts.
Our starting point is to connect with the goals and wishes of the client through collaboration with the client, their loved ones, and the chain partners. Treatment takes place in the client's own environment as much as possible, and is as short as possible and as long as necessary.
We work according to the FACT method: an intensive form of community, neighborhood-oriented treatment, recovery-oriented and with colleagues working together in a single multidisciplinary team.
Within FACT, work is carried out in multidisciplinary teams consisting of psychiatrists, nurse practitioners, mental health psychologists, general practitioners, community nurses/coaches, social workers, IPS job coaches, and trajectory coordinators.
As a community nurse, you support clients within a shared caseload. In consultation with the case manager, you carry out sub-tasks of the treatment. You visit clients during home visits with respect for their living/home environment or receive them at the location.
You contribute to the establishment of the treatment goals. You do this in consultation with the client and family members and/or the client's network and in collaboration with the case manager.
You are empathetic and have an eye for the client's full story. Together with them and their loved ones, you strive for recovery, no matter how small the steps may sometimes seem.
You are able to conduct home visits to clients independently or receive them at a location. During this, you make observations and signal changes/decompensation in clients and consult your colleague case manager and/or other disciplines for this.
You carry out sub-treatments from the legally mandated GGZ (such as administering a long-acting injection). You may also be asked by the case manager to prepare reports in the context of the WVGGZ process.
Performing office shifts is part of your duties; You will learn when a case manager or practitioner must be called in during a crisis situation.
You are able to work methodically and maintain an overview of the shared caseload.
You participate in the mandatory FACT training.
During the pilot, the position is scaled in FWG 45 according to the GGZ collective labor agreement. During the pilot, the definitive position will be described and weighed in accordance with the FWG methodology.
At GGZ inGeest, we support you in your development, whether it is about deepening expertise, learning new skills, or increasing digital knowledge. You can read how we do this in the general terms and conditions.
You will receive a fixed-term contract for a duration of 7 months. In the event of a successful pilot, there is the possibility that this contract will be converted into permanent employment.
