Community Nurse Wassenaar 24-32 hours

As a community nurse at Amarosa, you can truly make a difference in the home situation of clients. You will be given the space to take charge of care, coach colleagues, and work together in a close-knit and committed team. Thanks to our small scale, lines of communication are short and personal attention is central - for both clients and employees.

As a community nurse at Amarosa, you are the director of care at home. You assess needs, coordinate, and monitor the quality of care for clients who wish to continue living independently at home for as long as possible. You work closely with clients, informal caregivers, GPs, and colleagues, and you are an important link within the care network.

• Making assessments and drawing up, evaluating, and adjusting care plans;

• Taking charge of the care process for clients in the community;

• Providing nursing care and managing complex care situations;

• Coaching and supporting colleagues in their professional development;

• Participating in regional consultations and thinking along about how we organize community nursing to be future-proof;

• Working together with internal and external care partners to realize the best possible care.

You are a committed and expert professional who works independently and easily connects with clients, informal caregivers, and colleagues. You see opportunities to improve care and take initiative in doing so.

Furthermore, you possess:

• A Bachelor's degree (HBO) in Nursing and a valid BIG registration;

• Experience in home care or community nursing;

• Knowledge of assessment and care coordination;

• Good communication and coaching skills.
