The role

We are looking for two new colleagues to strengthen our team. We align care and well-being as much as possible with the resident, family, and the resident's environment, and work closely together to make things as pleasant as possible for the resident.

Where you will work

Your workplace is our De Drie Linden location. This is located at Westhovenplein in The Hague and consists of three small-scale residential buildings with 10 residents per floor. People with an indication of dementia live here, and there is a residential floor for people with an empathic-directive approach, such as Korsakov, and residents with other cognitive problems.

You will be working at the Zomerlinde. People with various forms of dementia live here.

What you will do as a Residential Care Coordinator

As a residential care coordinator, you will coordinate care and well-being around your residents together with your team and ensure continuity. During your shift, you are responsible for the individual client plan and are the person whom residents, family members, and staff can approach with questions. You coordinate the daily care provision within the team, you hold care plan discussions, and you are present at the MDO (multidisciplinary consultation).

The care and nursing of clients is, of course, part of your task as a residential care coordinator. In doing so, you adhere to the agreements made in the care plan. Naturally, you also keep an eye on the psychological well-being of your clients. You provide psychosocial guidance if necessary.

Additionally, you play an important role in the team in which you work. You stimulate cooperation, independence, and the growth of your team members, and you commit yourself to a good working atmosphere. You guide and support students and interns, playing a coaching and stimulating role in doing so.
