The role

Gewoon Thuis offers clients both planned and unplanned support in their own home environment, in collaboration with care partners, facilities, and the social network, so that they can live independently faster and longer. As a Personal Support Worker, you are the pivot in the care surrounding clients. You support them with daily matters, set goals together for independence, and actively communicate with colleagues, care partners, the municipality, and the client's network. In doing so, you deploy various forms of care, such as trajectory guidance, budget coaching, or remote care via video calling. We are looking for a flexible, driven colleague who stands firmly in their shoes, is empathetic, can work independently, and collaborates well. You provide both planned and unplanned care, are part of the on-call service (including evenings and occasional weekends), and can rely on the team and the safety net during intensive situations. Clients often have complex or psychiatric issues, including psychotic vulnerability. You possess basic knowledge and a theoretical background, can assess situations well, recognize signals, and act professionally.

In short: You are the connecting factor that supports clients in their independence, with an eye for their own agency and tailored care.

Your workplace

In Flevoland, Gewoon Thuis supports 25 clients. In Almere, you will work in a subteam of 5 employees within Team Outreach, which consists of approximately 20 care professionals. The team has various specializations, such as youth, family shelter, forensic care, and outreach care. Within the team, knowledge is actively shared and colleagues support each other with casework. The Gewoon Thuis subteam offers a soft landing for clients who are moving from sheltered housing to living independently. At the same time, we are committed to preventing admission to sheltered housing. All team members work day shifts, intermediate shifts, and on-call shifts, and collectively determine the organization of activities.

At Gewoon Thuis, you get the chance to truly make a difference, help build a new form of support, and grow in your own profession. Together, we make care personal, involved, and decisive.
