The role

Gewoon Thuis offers clients both planned and unplanned support in their own home environment. This takes place in collaboration with care partners, facilities, and the social network, so that clients can live independently faster and longer. As a residential support worker, you are the linchpin in the care surrounding the clients. You support them with daily matters, help them pursue their goals, and actively communicate with the personal support worker(s). You can accompany them to, for example, appointments at the GP, municipality, or administrator to support the client. Additionally, various forms of care are deployed, such as trajectory guidance, budget coaching, and remote care via video calling. You can also provide support in these areas or prepare conversations together with the client. We are looking for a flexible and driven colleague who is self-assured, empathetic, can work independently, and can collaborate well. Clients often deal with complex or psychiatric issues, including psychotic vulnerability. You possess basic knowledge and a theoretical background, can assess situations well, recognize signals, and act professionally in response.

In short: you are the connecting factor who supports clients in their independence, with an eye for their own autonomy and appropriate tailored care.

Your workplace

In Flevoland, Gewoon Thuis supports 35 clients. In Almere, you will work in a subteam of 5 employees within Team Outreach, which consists of approximately 20 care professionals. The team has various specializations, such as youth, family shelter, forensic care, and outreach care. Within the team, knowledge is actively shared and colleagues support each other with casework. The Gewoon Thuis subteam offers a soft landing for clients transitioning from sheltered housing to living independently. At the same time, we are committed to preventing admission to sheltered housing. All team members work day shifts, intermediate shifts, and on-call shifts, and collectively determine the organization of activities.

At Gewoon Thuis, you get the chance to truly make a difference, help build a new form of support, and grow in your own profession. Together, we make care personal, involved, and decisive.
