As a personal counselor, you are the fixed point of contact for a group of clients. You help them formulate personal goals that contribute to living as independently as possible. In doing so, you look not only at what someone needs, but especially at what makes someone happy. You conduct conversations that matter, offer practical support where necessary, and build a bond of trust that provides safety and stability. You coordinate the care surrounding your clients and work intensively with colleagues, external organizations, and the client's social network. You carefully monitor progress, evaluate regularly, and adjust where necessary. The Tiendweg West location is a rural location in a beautiful spot in Lekkerkerk. The location is a farmhouse with 21 residential apartments and 2 guest places. Additionally, the activation center is also located on our premises, which is part of Kwintes and offers activation to both Kwintes clients and external parties. Our residents all live independently in their own apartment. The guidance is tailored to the wishes and needs of the resident. The target group consists of adults with psychiatric vulnerabilities. You know what people need to take steps in their recovery, and you switch effortlessly between client contact, collaboration, and coordination. You bring knowledge as well as the drive to put that into practice. Furthermore:

• You have completed a relevant HBO (higher professional education) degree, such as Social Work, SPH, or Nursing;

• You have experience in mental healthcare (GGZ) and know how to guide clients through psychiatric and psychosocial challenges;

• You are familiar with the rehabilitation methodology and apply it purposefully in your guidance;

• You coordinate the care process and coordinate with colleagues and chain partners, clearly dividing tasks;

• You actively build a network around the client and maintain contact with stakeholders such as family, GPs, and municipalities;

• You work together with colleagues toward a shared goal and are open to new insights and ways of working.

• A good salary between €3,180 and €4,273 based on 36 hours (CAO GGZ, FWG 45).

• Your first contract is a temporary contract. If we are happy with each other, your next contract will be for an indefinite period.

• In addition to the statutory 8.33% holiday allowance, you receive an end-of-year bonus of 8.33%, and at Kwintes, we offer a balance budget of €1,000 gross per year pro rata to your employment.

The procedure consists of 2 interviews; the 1st interview is with the recruiter and the 2nd interview is on-site with 1 or 2 colleagues and a client. If applicable, a shadowing moment and the employment conditions interview will follow. Upon employment, we request a Certificate of Good Conduct (VOG).
