Individual Healthcare Provider (Verzorgende IG) Hoek van Holland

As an Individual Healthcare Provider, you provide personal care and perform nursing technical procedures, such as administering medication. You look at what a client or resident can and wants to do themselves and pay attention to the person behind the care request.

In Hoek van Holland, you can work at clients' homes and in our nursing home. This allows you to combine the independence of community care with working in a fixed team. Prefer working alone in the community or in the nursing home? That is also open for discussion.

You identify changes in health and well-being, draw up care plans, execute them, and report on them. You work together with colleagues and other disciplines and make time for personal contact.

Do you have EVV/contact caregiver tasks? Then you coordinate the care surrounding residents, monitor agreements from the care plan, and maintain contact with residents, loved ones, and colleagues.

Your work area is Hoek van Holland. Stichting Humanitas offers both community care and nursing home care here. You will deal with various care requests, including somatic and psychogeriatric issues and dementia.

In the community, you help clients to live at home independently and comfortably for as long as possible. You work independently, have freedom during your route, and visit various clients at home.

In the Bertus Bliekhuis nursing home, you provide care in the familiar living environment of the residents. You work closely with colleagues and other disciplines and have a fixed team around you.

• Salary according to CAO VVT FWG 35/40 (€17.48 - €24.69 gross per hour / €2,735.91 - €3,864.43 based on 36 hours per month);
