Do you recognize yourself in this and do you want to use your nursing expertise to guide patients toward the best appropriate follow-up care? Then we would love to meet you!

The role

Do you make the difference at the moment a patient leaves the Mumc+? As a transfer nurse, you play an important role in the transition from hospital care to appropriate follow-up care. No person is the same, and every patient brings their own, unique care needs and situation. It is precisely that variety that makes your work meaningful and challenging

With your nursing expertise, up-to-date knowledge of laws and regulations, and an eye for the interests of the patient and their loved ones, you manage a careful and appropriate follow-up trajectory time and again.

You carry out your duties independently and objectively and support the hospital departments with complex issues regarding aftercare. Your duties include, among other things:

What you'll do

  • Organizing, coordinating, indicating, and advising on appropriate follow-up care;

  • Informing and guiding patients and their loved ones regarding the possibilities for aftercare;

  • Assessing and administratively processing requests for follow-up care;

  • Providing advice and consultation in complex cases;

  • Maintaining contacts with internal and external care partners, such as the CIZ, care providers, and home care organizations;

  • Acting as an expert in the field of relevant laws and regulations.

In addition, you contribute to the development and promotion of integrated care in the region. You participate in multidisciplinary consultations, regional and national working groups, and various training activities.

What do we offer?

  • A varied position in the complex world of aftercare;

  • An expert and dedicated team;

  • Space for training and personal development;

  • Enjoy a lot of independence and responsibility;

  • The opportunity to think along about improvements within transfer care.

At Maastricht UMC+, we ensure that you have everything to make your contribution to ultimate care. The right employment conditions belong with that. Read here what we offer you.
