The role

Appropriate care for your clients

Westerpoort 4 is a closed step-down clinic for adults with long-term, severe psychiatric disorders (SMI). The clinic offers recovery-oriented care in a green environment. The focus lies not only on symptoms, but especially on the person, their network, and recovery needs. The goal is to work towards an appropriate discharge, such as protected living, (assisted) independent living, or another clinical facility.

Our intensive, long-term treatments are aimed at stabilizing the clinical picture, learning to deal with limitations, and increasing independence. In this, we work according to the ART methodology (Active Recovery Triad), in which the client works together with the treatment team on recovery, structure, and perspective.

As team coordinator, you play an important role in safeguarding the quality and continuity of care. You support colleagues in complex care situations, think along about treatment trajectories, and together with the multidisciplinary team, monitor the progress of care.

You work closely with psychiatrists, nurse practitioners, physicians, psychologists, nurses, caregivers, occupational therapists, social workers, and other disciplines. Additionally, you maintain contact with loved ones, chain partners, and follow-up facilities to realize the best possible care for the client.

A suitable working environment for you

As team coordinator, you combine direct client care with a coordinating and coaching role within the team.

You provide functional leadership during the shift and are the first point of contact for colleagues, practitioners, and managers. You monitor the daily processes on the ward, maintain an overview of personnel staffing, client care, and work distribution, and ensure a smooth flow of daily activities.

You coordinate the inflow and outflow of clients, monitor the progress of treatment trajectories, and ensure that important information is coordinated in a timely manner within the multidisciplinary team.

Additionally, you coach and guide colleagues in their daily activities. You support them in complex situations, stimulate professional development, and contribute to a safe working environment in which collaboration, learning, and reflection are central.

You identify opportunities for improvement within the ward, translate developments in care into practice, and make an active contribution to quality improvement and the further development of recovery-oriented care within Westerpoort.

Naturally, you also remain involved in client care yourself and work on the ward where necessary.

Also important

  • You possess a completed relevant HBO-healthcare education, such as HBO-Nursing, Social Work (care profile), or SPH. For nurses, a BIG registration is a plus.

  • You have several years of work experience within mental healthcare, preferably with clients with severe psychiatric disorders (SMI).

  • You have experience with the functional management and coaching of colleagues.

  • You have knowledge of recovery-oriented working and an affinity with the ART methodology.

  • You are decisive, organizationally strong, and maintain an overview in complex situations.

  • You are connecting, communicatively skilled, and know how to positively involve colleagues in changes.

  • You are willing to participate in direct client care.

  • The position is scaled in FWG 55 in accordance with the GGZ collective labor agreement (CAO).

  • Good employment conditions, including ample opportunities for schooling, development, and career growth within GGZ inGeest.

  • You will receive a permanent employment contract immediately.
